• steps to reproduce the bug
• when does it happen?
  • spontaneously during normal gameplay
  • immediately after loading a particular mod
  • when adjusting the character via the in-game sliders?
• which Loader mods are you using?

The simplest explanation is that you have a Loader mod which replaces the breasts with [breast + clothing] or [reshaped breasts] or [armored plate which is too small for normal breasts to fit behind it] or whatever. When this mod is unloaded, the new content is removed but the original breasts do not reappear -- because the mod author was a bit lazy and didn't bother with cleanup. Thus, the girl is left with a chunk missing from her torso.

"How to fix?"
If it's caused by a mod, then ask the mod author to fix it. Or stop using that mod.
As a temporary workaround, just reload the game (Alt+F, 1).

How it happens with me is if I save a character, while also having either one of Dante's head mods on while saving that character, that is what happens.

You're talking about Dante's male heads? Don't use the in-game save, because it doesn't work properly with the loader.
