Have you edited the settings file? If so, send a copy of it in a PM and I'll take a look at it.

That usually happens if PenisRange isn't installed and loaded. You can find a link to it at the top of the Overview page of this mod. If that doesn't fix it, send me a PM and I'll see what I can do.

The auto scaling is optional and is on by default.
To turn it off, change the setting "herHeadScaleAuto" from 1 to 0.
When it is off, you should be able to scale her head with the keybindings.

I agree that her head looks too small when you scale it down with auto scaling enabled, I should probably make it scale down "slower".
I think that I should also make the head scale keybinds should change the "relative scale" when auto scaling is enabled (instead of doing nothing).

That error appears when it fails to find the settings file. The settings file should be placed inside "Settings" folder (it is next to files like "Loader.swf" and "SDT.swf").

You can change the size by pressing the scale up/down buttons (the default keys are listed on this page).

I'm don't have access to my computer atm so I can't check it right now, but I think that I already fixed that issue. Could you PM me a more detailed description of the issue (screenshots would also be nice)? : )
