The basic mechanics of this scene can be setup fairly easily in Animtools. There are a few scripting tasks which would be needed in order to complete it:

• layering - put her body between his legs
• layering - put her face (incl. tongue) between his buttocks
• graphics - allow his feet to rotate to a custom angle (rather than pointing his soles directly towards the floor) so that he can kneel properly
• graphics - extend the IKController logic so that he can kneel properly
• scripting - detect close proximity so that her dialog can be automatically muffled when she's face-deep in butt
• narrative - write a dialog which assembles the position files and scripts into an actual gameplay scenario
