To get to the first ending(you end up alone).

When you see the hitchhiker do not stop.
To get to the second ending (you end up alone, but want to find Marcella).
When you talk to Frank, agree to stop drinking.
Pick up the hitchhiker.
Compliment her on her breasts.
Ask about Marcella's accent.
To get the third ending (you end up with the doctor).
When you talk to Frank, agree to stop drinking.
Pick up the hitchhiker.
Do NOT compliment Marcella on her breasts.
